Salary: $16.23 – $20.29 Hourly. Commensurate with experience.

Essential Functions
Provides case coordination in a variety of settings including office, home, school and community. Coordinates wraparound team meetings, multiple community support services and authorizes services included in the plan of care. Ensures that families experience voice, access and ownership of the plan of care, consistent with the wraparound model. Maintains a caseload of 12 families with a minimum of 1 hour of direct contact per week to assigned families. Works with children returning to the community from Child Protective Services facilitated placements. Provides transition services and linkage to community resources and supports with a strong emphasis on mental health and school related services.

Required Qualifications
Education and experience equivalent to a Bachelor’s degree in one of the social or behavioral sciences AND two (2) years experience in social services related work.

License: Possession of a valid Texas Driver’s License may be required.

Grant Funded until 11/30/10 and may be renewed

Preferred: *Bilingual in English/Spanish *Knowledge of Wraparound Approach and System of Care; skill in facilitating team meetings; knowledge of local resources; completion of cultural competency training(s); experience in children’s mental health and knowledge of child serving agencies particularly Child Protective Services.

Hours: *8:00 AM to 5:00 PM and works some nights and weekends. *Must meet with families at times and locations that best suit the family’s schedule. This requires visits to occur in the evenings and on weekends.

Work site: *100 North IH 35, Palm Square Building *May visit youth in Child Protective Placements located outside the Austin area.
